Virtual Care Team™ is the heart of the iCare telehealth system. It is the central tracking and communication application that is delivered to your organization via ASP, application service provision. Your instance of Virtual Care Team will be configured for your organization with your own features, functions, and branding to serve your clinical goals. ASP means that the application runs on our servers...saving you start up time and costs...saving you maintenance and support headaches. Select the features and functions that your organization needs:

  • Database- the industrial grade ODBC database contains information about each user, patient, clinician, health events (vital signs, medication regimen, etc), communication preferences, alerts.

  • Care Plan Templates- allows easy set up of the individual patient with your best practice guidelines.

  • HL7 interface- to your clinical data sources

  • Rules Engine- monitors vital signs, medication, and other health events and triggers alerts to the care team

  • Intelligent Health Messenger- controls and monitors electronic alerts and communication between care team members via several communication modalities- mobile phone, pager, e-mail, e-Fax, e-Letter, video- configurable to the individual user.

  • Cascading Alerts- If a care team member does not respond to an alert within a predetermined time, the Intelligent Health Messenger sends the alert to another care giver or to the call center.

  • Professional Med Page- is the web portal through which the clinicians monitor their patients and manage Virtual Care Team communications

  • Your Personal Health Coach- is the optional web portal for patient and family that provides a lay person view of patient data, educational material specific for patient problems and health information resources. Your Personal Health Coach can coach patient and family care givers in better self care in accordance with the care regimen that your clinicians have established.

Telehealth ISP™ is a collection of internet connection technologies that iCare uses to condition local internet connections to match the right technology with your clinical needs and to meet the unique demands of healthcare, such as HIPAA, security, cost. 

Remote Telecare Services comprise the menu of clinical services that your iCare system will provide to your  remote patients. You can customize iCare for the individual patient and change each patient's services as their clinical needs change. Remote telecare services can include one or all of:

iCare Video Housecall™- simple, easy, inexpensive one button telemedicine provides remote site medical quality digital video with the iCare ExamCam™. iCare Video Housecall™ system can be configured for video house calls, nursing home rounds, clinic-to-clinic tele-consultation, plus tele-coaching and education of remote care givers.

Vital Sign Telemonitoring- multiple methods of remote data capture, any one or all of which can be used in one system, customizable to individual and care giver need- automatic upload of physiologic data via regular telephone, self report of data via interactive voice response, self report via family web portal (Your Personal Health Coach), cell phone, pager, or e-mail. 

Medication Dispensing and Monitoring- automated reminders to take medications, automated dispensing of medications, automated report of missed medications.

Personal Emergency Response System- basic and advanced PERS with call center support, call center triage, and optional telephone social contact.
